Submit Data/Upload Image
ACR Accreditation requires electronic upload of all accreditation images and documents. Electronic submission reduces costs, ensures compliance with HIPAA regulations and speeds turnaround time from image submission to final results.
05
Gather Data
Please use these documents and forms to gather data to be entered into the online testing package before sending images for review. Please note that these forms should not be submitted to the ACR for accreditation.
Image reviewers requested that sites submit digital images in JPEG, GIF, TIFF or BMP format for both clinical and phantom images if possible. If reviewers experience difficulty with a CD submission, they will return them and this will result in a significant delay of your accreditation.

Prepare for Site Visits
All outpatient facility providers of CT, MRI, breast MRI, nuclear medicine, and PET exams that bill under Part B of the Medicare Physician Fee Schedule must be accredited in order to receive payment for the technical component of these services. The ACR will perform unannounced site surveys on accredited facilities to validate compliance with accreditation criteria. For facilities that fall under MIPPA, these site visits will be unannounced. We strongly advise sites to use the documents below to gather and organize information for these site surveys.
